ABSTRACT

We aimed to study radiomics approach based on biparametric magnetic resonance imaging (MRI) for determining significant residual cancer after androgen deprivation therapy (ADT). Ninety-two post-ADT prostate cancer patients underwent MRI before prostatectomy (62 with significant residual disease and 30 with complete response or minimum residual disease [CR/MRD]). Totally, 100 significant residual, 52 CR/MRD lesions, and 70 benign tissues were selected according to pathology. First, 381 radiomics features were extracted from T2-weighted imaging, diffusion-weighted imaging, and apparent diffusion coefficient (ADC) maps. Optimal features were selected using a support vector machine with a recursive feature elimination algorithm (SVM-RFE). Then, ADC values of significant residual, CR/MRD lesions, and benign tissues were compared by one-way analysis of variance. Logistic regression was used to construct models with SVM features to differentiate between each pair of tissues. Third, the efficiencies of ADC value and radiomics models for differentiating the three tissues were assessed by area under receiver operating characteristic curve (AUC). The ADC value (mean ± standard deviation [s.d.]) of significant residual lesions ([1.10 ± 0.02] × 10-3 mm2 s-1) was significantly lower than that of CR/MRD ([1.17 ± 0.02] × 10-3 mm2 s-1), which was significantly lower than that of benign tissues ([1.30 ± 0.02] × 10-3 mm2 s-1; both P < 0.05). The SVM feature models were comparable to ADC value in distinguishing CR/MRD from benign tissue (AUC: 0.766 vs 0.792) and distinguishing residual from benign tissue (AUC: 0.825 vs 0.835) (both P > 0.05), but superior to ADC value in differentiating significant residual from CR/MRD (AUC: 0.748 vs 0.558; P = 0.041). Radiomics approach with biparametric MRI could promote the detection of significant residual prostate cancer after ADT.

ABSTRACT

OBJECTIVE:To provide reference for chronic disease man agement service develophed in social pharmacy. METHODS:Questionnaire about the Status Quo of Chronic Disease Management and Service in Social Pharmacy of Chengdu was designed,using the quota sampling method ,social pharmacies in five main urban areas of Chengdu were selected from May to July,2018 to conduct a questionnaire survey (one questionnaire by each social pharmacy ) on the basic situation of social pharmacies,the development of chronic disease management services ,the cognition of chronic disease management services ,and the challenges faced by chronic disease management services ,and suggestions were proposed. RESULTS & CONCLUSIONS :A total of 272 questionnaires were sent out ,and 252 valid questionnaire were actually collected (effective recovery rate of 92.65%). Totally 189 sample pharmacies (75.00%) had carried out chronic disease management services ,of which 112 (59.26%) pharmacies had been launched for 1-3 years;87(46.03%)had set up service areas ;68(35.98%)had full-time staff ,and 54 (28.57%)had part-time staff ,most of which were licensed pharmactists. 116(61.37%)had trained related staff for 1-2 times per year. 176(93.12%)pharmacies could provide services such as basic indicator testing (176,93.12%),establishing health records (142,75.13.%),and rational medication guidance for patients (163,86.24%). According to the survey ,the substantial benefits of chronic disease management services included changing the health status of patients (163,86.24%),improving patients ’trust in the pharmacy and staff (141,74.60%),improving patients ’quality of life (129,68.25%),etc. More than 50% of pharmacies faced the challenges of limited number of licensed pharmacists (102,53.97%),difficulty in establishing professional teams (112, 59.26%),and lack of trust in services (101,53.44%). The current chronic disease management service of social pharmacy in Chengdu is in the initial stage of active exploration ,which can bring many benefits to patients and pharmacies ,and is conducive to the promotion of medical and health policies ,but there are some weak links at the same time. ABSTRACT

The aims of this study were to determine the prognostic value of primary tumor surgery and identify optimal candidates for such surgery among patients with seminoma and distant metastasis at diagnosis. We identified 521 patients with seminoma and distant metastasis at diagnosis between 2004 and 2014 from the Surveillance, Epidemiology, and End Results database. Among these patients, 434 had undergone surgery, whereas 87 had not. The prognostic value of primary tumor surgery was assessed by Kaplan-Meier methods, log-rank analyses, and multivariate Cox's proportional hazards model. Survival curves and forest plots were also plotted. Survival analysis indicated that patients who underwent surgery had a better 5-year overall survival and cancer-specific survival than those who did not. Multivariate analyses demonstrated that primary tumor surgery is an independent prognostic factor for overall survival and cancer-specific survival, along with age at diagnosis, M stage, and marital status. In addition, primary tumor surgery still had considerable prognostic value in the subgroup of patients with lymph node metastasis. Further, forest plots demonstrated that patients with M1a stage, N1 or N2-3 stage, and a younger age at diagnosis (<60 years) may benefit from primary tumor surgery. In conclusion, our findings indicate that primary tumor surgery is correlated with improved survival in patients with seminoma and distant metastasis. Furthermore, primary tumor surgery is an independent prognostic indicator for patients with seminoma and distant metastasis.

ABSTRACT

Objective To investigate the different concentrations of progesterone on the expression of a disintegrin and metalloprotease 10 (ADAM10),long form leptin receptor (Ob-R) and the secretion of soluble leptin receptor (SLR),leptin (LEP) in primary pregnancy human chorionic trophoblast cells. Methods Cultured primary human trophoblast cells and added in different concentrations of progesterone (0,100,150 and 200 ng/mL) for 24 hours. The relative expression of ADAM10 and Ob-R in the cells and the content of SLR and LEP in the supernatant were detected. Results With increasing concentrations of progesterone,early human trophoblast cells ADAM10 content gradual-ly decreased,the difference between the two groups was significant (P0. 05). SLR content of the cell supernatants as the concentration of progesterone increased and decreased,there are significant differences between each two groups (P>0. 05). LEP cell supernatant in each group with the increase of the concentration of progesterone concentration increased gradually between the two groups were significantly different(P>0. 05). Pearson’s test showed that the expression of expression SLR and LEP exists a significant negative correlation (R= -0. 949,P<0. 01). Conclusion Progesterone may influence the ex-pression of ADAM10,SLR and LEP by the regulation of leptin to participate GDM occurs.

ABSTRACT

<p><b>OBJECTIVE</b>To study the expression of neonatal Fc receptor in podocytes in human nephritis and immune-induced rat nephritis models: anti-Thy1.1 nephritis and Heymann nephritis.</p><p><b>METHODS</b>Thirty-nine cases of renal biopsies were enrolled from September 2009 to February 2010, including 8 cases of minimal change disease, 4 cases of focal segmental glomerulosclerosis, 9 cases of membranous nephropathy, 12 cases of IgA nephropathy and 6 cases of lupus nephritis. Five normal kidney tissue samples adjacent to renal clear-cell carcinoma were served as normal controls. Laser capture microdissection and real-time RT-PCR were used to assess the expression level of FcRn mRNA in glomeruli of various glomerulonephritides, and immunohistochemistry (IHC) of FcRn by SuperVision method was performed. In addition, rat models of mesangial proliferative nephritis (anti-Thy1.1 nephritis) and passive membranous nephropathy (Heymann nephritis) were established and FcRn was examined in renal tissues by IHC.</p><p><b>RESULTS</b>The FcRn mRNA level in lupus nephritis was statistically higher than that of normal controls (P < 0.05). FcRn protein expression by IHC was seen in lupus nephritis (6/6), membranous nephropathy (6/9) and IgA nephropathy (7/12), significantly higher than that of normal controls (0/5), P < 0.05. Minimal change disease and focal segmental glomerular sclerosis showed minimal or none expression of FcRn (1/8, 0/4 respectively) and not statistically difference from that of normal controls. Furthermore, FcRn expression in podocytes was detected in rat anti-Thy1.1 (3/5) and Heymann nephritis models (2/7) but was not detected in normal controls.</p><p><b>CONCLUSIONS</b>Expression of FcRn in podocytes was up-regulated in immune-induced human nephritis and rat nephritis models of anti-Thy1.1 nephritis and Heymann nephritis. FcRn may play a role in the development of immune-induced glomerulonephritis.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To develop a new injectable biomaterial carrageenan/nano-hydroxyapatite/collagen (nHAC/Carr) for bone surgery and characterize it.</p><p><b>METHODS</b>nHAC/Carr was developed by filling carrageenan with nano-hydroxyapatite/collagen (nHAC) granules.</p><p><b>RESULTS</b>It was found that nHAC/Carr had similar X-ray diffraction patterns with that of nHAC. Fourier transform infrared spectroscopy showed that carrageenan did not change its structure in nHAC/Carr. The rheological behavior of nHAC/Carr was the same as carrageenan. The scanning electron microscopy and porosity analysis showed that nHAC/Carr had porous structure and its porosity was about 90%.</p><p><b>CONCLUSION</b>The biomaterial nHAC/Carr may be used as an injectable bone substitute.</p>

ABSTRACT

Objective To evaluate the inhibiting effect of antisense vascular smooth muscle alpha adenovirus vector on tubular epithelial myofibroblast transdifferentiation induced by TGF-?1.Methods The pAdanti-?-SMA was constructed and identified.The ?-SMA expression in tubular epithelial cells(TEC) was detected by immunofluorescence and Western blot.Five groups were designed as control(A),TEC transfected by 2 ?l pAdanti-?-SMA(B),TEC induced by TGF-?1(C),TEC induced by TGF-?1 and transfected by 2 ?l pAdanti-?-SMA(D),TEC induced by TGF-?1 and transfected by 10 ?l pAdanti-?-SMA(E).Results The pAdanti-?-SMA was successfully constructed.The expression of ?-SMA in TEC induced by TGF-?1 was inhibited by pAdanti-?-SMA.Conclusion Tubular epithelial myofibroblast transdifferentiation can be inhibited by pAdanti-?-SMA.
